Note:
I. The current voluntary group insurance plan of this Administration is underwritten by Cathay Life Insurance according to the contract until March 31, 114 (24:00). As per Article 4, Paragraph 4 of the contract for the Administration's voluntary group insurance from 112 to 114 (hereinafter referred to as the Contract), Cathay Life Insurance agrees to continue underwriting the Administration's voluntary group insurance from 114 to 116 with the current plan and premiums. The coverage period will be from April 1, 114 (00:00) to March 31, 116 (24:00), for a duration of 2 years.
II. Related matters are explained as follows:

(I) The Administration's voluntary group insurance plan refers domestic life insurance companies that are legally registered and willing to offer preferential terms to the Administration's employees. The insurance benefit funds are to be raised by the underwriting insurance company, and the policyholder is responsible for the full insurance premium. The Administration will not bear any subsidy.

(II) Any disputes arising from the Administration's voluntary group insurance plan shall be resolved by the underwriting insurance company and the policyholder in accordance with the Civil Code, Insurance Act, and relevant laws and regulations. The Administration will not be involved in the resolution.
III. For those who are still participating in the Administration's voluntary group insurance plan on March 31, 114, if they receive the renewal notice, agree to renew, sign the renewal documents and return them to Cathay Life Insurance, and the payment is successfully deducted, the insurance effective date will be retroactively effective from April 1, 114, without affecting the rights and interests of colleagues. The Administration's Department of Personnel will notify the heads of all first-level agencies (organizations) and district offices to send representatives to collect the renewal notices after Cathay Life Insurance submits them,

and distribute them to colleagues in their subordinate agencies (organizations) and schools.
IV. Attached are one copy each of the 114 Annual Protection Plan Summary, the list of service personnel at Cathay Life Insurance's agency windows, and a Q&A document.
